Therapy
Infliximab
Monoclonal antibody targeting TNF.
Also called: infliximab, Remicade, IFX

In short
- 1What it is: a therapy in therapies and applications.
- 2How it connects: acts on 4 and is affected by 2 concepts — mostly 1 molecule, 2 disease, 1 therapy.
- 3Where it sits: therapy, trial on the 15-step immune chain.
What it does
Chimeric IgG1; binds soluble AND membrane TNF → reverse signalling → macrophage/T cell apoptosis in granuloma → fistula healing in CD (etanercept cannot); ATTRACT trial (2000): first biologic to show structural protection in RA; risk: TB reactivation (mandatory LTBI), infusion reactions (slow first infusion), anti-drug antibodies (minimised by concomitant MTX); TDM: trough >3-7 μg/mL for mucosal healing in IBD
