Biomarker
ACPA
Anti-citrullinated protein antibodies: IgG directed against citrullinated proteins (fibrinogen, vimentin, α-enolase, collagen) generated by PAD2/4 enzymes in inflamed synovium.
Also called: anti-CCP, anti-cyclic citrullinated peptide, ACPA, anti-citrullinated protein

In short
- 1What it is: a biomarker in research methods.
- 2How it connects: acts on 1 and is affected by 2 concepts — mostly 1 disease, 1 therapy, 1 biomarker.
- 3Where it sits: biomarker, failure on the 15-step immune chain.
What it does
High-titre ACPA has been linked to better abatacept response in observational cohorts; guidelines do not mandate a specific biologic; ACPA titre falls with rituximab (PD biomarker in RAVE-like trials confirming B cell → ACPA chain)
